Our Clarence Care + Support nurses are available to consult on a clinical need or to come to your home to provide direct nursing services in the comfort of your own home.
If over 65 years or ATSI and over 50 years, contact My Aged Care on 1800 200 422 and register, then they will organise for an assessment by the Aged Care Assessment Team (ACAT) or the Regional Assessment Service (RAS). You can then request which organisation you would like to work with you.
If under 65 years and looking for NDIS planning contact NDIS on 1800 800 110.
Veterans need to contact the DVA number 1300 550 450 to arrange for an assessment first. If already eligible you can request for our service to work with you.
